Disability Achievement Center To Offer COVID Vaccines for All with a Disability

Florida Centers for Independent Living (CIL) have partnered with Florida Department of Health (FLDOH) to offer COVID-19 vaccines to any person with a disability.  Disability Achievement Center is the CIL that serves Pinellas and Pasco Counties.  We are located at 12552 Belcher Rd S, Largo, FL  33773.

The Johnson & Johnson vaccine is available to anyone with a disability and we will provide free transportation within Pinellas County (through PSTA) if needed. We invite you to take advantage of this unique opportunity.  You can be placed on our registration list by filling out the CIL form (below) and returning it to us either via fax (727) 539-7588 or by email: jodya@mydacil.org.

A physician completed “COVID-19 Determination of Extreme Vulnerability” is necessary for anyone with a disability under age 60.  The form is below.  Disability Achievement Center’s fax number is 727-539-7588 if the physician desires to fax the completed form directly to us prior to vaccine administration.

A Heartfelt Testimony of How DAC Helps

My wife Carol has received MANY years of support from the Disability Achievement Center. And Vickie P. has been the one who spearheaded the initial contact, followed up on support services and remains our point of contact for continuing supplies.
I can remember during the VERY EARLY stages of Carol’s mobility decline, before the ALS diagnosis, Vickie worked to help us install tub rails and later a rail for our front stairs. Now her continual support over the years with NEEDED briefs is a HUGE help. What an outstanding team of good people. They have always been helpful, supported and continue to smile. Thank you Vickie and all. We are very grateful to your organization.

Please Share New Vaccine Information!

Good Morning,

We would like your assistance in increasing awareness of four new COVID-19 vaccination sites being established in partnership with the federal government in Miami, Orlando, Tampa, and Jacksonville. These sites will open on March 3, 2021 and will operate 7 days per week, 7 a.m. – 7 p.m. Each site will administer 2,000 vaccines per day at the locations listed below. Additionally, each site will have two smaller, mobile satellite sites that will each conduct 500 vaccinations per day in underserved areas. In addition, Executive Order 21-47 was filed yesterday. Effective March 3, 2021, individuals eligible to receive COVID-19 vaccine include:

  • Long-term care facility residents and staff.
  • Persons 65 years of age and older.
  • Health care personnel with direct patient contact.
  • K-12 school employees 50 years of age and older.
  • Sworn law enforcement officers 50 years of age and older.
  • Firefighters 50 years of age and older.

Please share this information with your clients, partners, and communities and encourage eligible individuals to preregister with the statewide preregistration system by either calling each county’s designated phone number – complete list of phone numbers available on the COVID-19 Vaccines in Florida webpage or visiting MyVaccine.FL.gov. Onsite walk-up registration will also be available.
